【Initial appointments】

• We are currently not accepting new patients. Appointments for initial consultations in February 2025 will be available starting from 14:30 on January 15th.
• If you have already completed an initial appointment, please print and complete the Intake Form, and bring it with you on your first visit.

【9月以降の（金）18:00〜19:00の予約について】

• 「待たずに診察を受けたい」というご要望にお応えして、9月以降は金曜日の18:00〜19:00を特別予約枠と致します。15分の枠に1名のみの予約となるため、待たずにゆっくりと診察を受けたい方は、ご活用ください。
  （予約料として別途5,000円＋税がかかりますので、ご了承ください）
  
• また、この変更に伴い、金曜日の通常診療は18:00まで（受付は17:45まで）となります。
  ご理解のほどよろしくお願い申し上げます。

【Regarding Reservations on Fridays from 6:00 PM to 7:00 PM Starting in September】

• In response to requests to "receive a consultation without waiting," we will be offering a special appointment slot on Fridays from 6:00 PM to 7:00 PM starting in September. Since we will only accept one reservation per 15-minute slot, those who wish to receive a consultation without waiting and in a relaxed manner are encouraged to utilize this service.
  (Please note that an additional fee of 5,000 yen plus tax will be charged for this reservation.)
• Due to this change, regular consultations on Fridays will end at 6:00 PM (reception will close at 5:45 PM).

• オンライン資格確認は設置しておりますが、初診時にはマイナンバーカードと保険証をお持ちください。

• 必要でない限り、友人やご家族と一緒に来院しないでください。

• If you have any cold-like symptoms, please do not come to the clinic and call us instead.
